*Factoid: When a pearl adheres to the shell of a mollusk, it's left to gestate for longer. They're more rare & valuable than most - particularly, when very lustrous. If it's harvested with the shell (like the bottom ones), it's called a "Blister Pearl". When harvested without the surrounding shell, it's called a "Mabe" (Pronounced 'Mah-BAY'. Like the top ones).
Typically, when Mabe & Blister pearls are used in jewelry, they leave the backs open - so you can see the shell - to prove that they're real. In this case, the backs of the top component have been covered - encased in a bezel setting, to support the post. ;)
